Below you will find two registration forms, the first is our standard form which you may use to register a participant and a spouse. We received a request for a participant only because some educational administrator's may object to see spouses on the form. So, you may use either form, but if you use the participant only form and you are bring a spouse, you will need to use both forms, the first for the spouse and the second for the participant.
